Recommendation Letter - the recommendation letter form must be filled out if a recommendation letter is needed from Mrs. Gainard. The form helps ensure I can write about all of your accomplishments.

Transcript Request Form - this form must be filled out and submitted to the Guidance Office in order for a transcript to be sent out to a college, university, tech/trade school, job, or military. *This is only for CURRENT students.  If you are a former student who needs their transcript sent somewhere, please contact Angela Bartlett at abartlett@windham-schools.org or by phone at 330.326.2711 x515

• One form must be filled out for each place a transcript is sent to.

College Visit Request Form -  This form must be signed by every teacher, Mrs. Gainard, and turned into the Attendance office before the college visit takes place.
