DENTAL VAN COMING TO WINDHAM (Click for Details)

"My school dentist" has the dental van coming to KT on 9/16 and 9/17 as well as the JH/HS high on 9/17.

Parents can go to:

To register their students. They need to do is go on the website enter the zip code (44288) and pick KT Elementary or Windham JH

Even though the high school is not listed, they will see high school students, please just register under the JH.

The nurse at KT will be sending home a paper registration form at KT with all students on the first day.

I also have paper forms that will be available in the Nurses office at JH/HS by request.

Forms will be accepted all the way up to the day of the visit, but earlier is better! 

Medicaid insurance is accepted!
