Attention 6th and 11th grade students and parents:

The State of Ohio requires incoming 7th and 12th graders to have vaccinations before the start of the 2020-2021 school year.  Seventh graders need a TDAP and Meningococcal vaccine, and twelfth graders need a second Meningococcal vaccine.  These are provided free of charge and conveniently during the school day.  Please fill out the form below and send it back to the school before March 16th.
