Health Heroes, in partnership with Akron Children's Hospital, is offering on-site flu vaccinations to students, staff, and community members of Windham. The clinic will be at the JR/SR High School on Wednesday, September 20 from 8:00 AM-3:00 PM. In order to receive a vaccination, the forms below must be completely filled out prior to the clinic date to ensure enough doses are available. Forms are to be submitted to the JR/SR High main office. If you need additional copies of the forms, please stop in the main office or call 330-326-2711. If you have any questions or concerns about the vaccination, contact Health Heroes at 1-205-609-0268 or al@healthheroes.com.  Students will receive a copy of the information to bring home.
